Clinical Pharmacy

The Clinical Pharmacy Program provides continuous access to the expert knowledge of a Clinical Pharmacist through direct collaboration with family physicians and the interdisciplinary team. Program Objectives include:

  • To advance the implementation of effective pharmacotherapy in primary care through the direct service and consultation of a Clinical Pharmacist
  • To provide individual patient assessments to identify, prevent and resolve drug-related problems
  • To provide patient education for the initiation or modification of significant or complex medication regimes
  • To support the integration of evidence-based pharmacotherapy in C-FHT chronic disease management programs
  • To promote utilization of the MedsCheck Program in community pharmacy settings for eligible patients to support consistent understanding of medication regimes among patient, dispensing pharmacy and family health team
