Adult Mental Health
The Adult Mental Health Program provides support to patients who are identified to have mental health needs. Assessment and interventions are provided by a team of social workers, a clinical pharmacist, with leadership provided by a FHT physician with expert knowledge in mental health and consultation provided by a sessional psychiatrist. Program objectives include:
- To assess the mental health status and support needs of all patients referred to the program by a C-FHT physician or interdisciplinary team member
- To establish an appropriate plan of care for patients registered to the program
- To promote consideration of mental health needs as a standard of practice at the C-FHT
The Adult Mental Health Program provides the following services to patients within The Caroline Family Health Team of Burlington:
- Clinical assessment
- Treatment (including but not limited to Cognitive Behavioural Therapy and pharmacologic therapy)
- Disease support
- Education
- Referral to community services
- Ongoing care and support for patients with persistent illness to prevent deterioration and potential hospitalization
