FAQ
– As a policy, we do not accommodate walk-in patients, and request that you call first to make an appointment. We only service patients registered to our clinic who have a scheduled appointment.
– Please note that the C-FHT is not a walk-in clinic. Therefore patients with family doctors elsewhere should consult their own doctors.
If you are unable to attend a scheduled appointment, please phone the office the day before to cancel. Other patients need your appointment time. Please be considerate. Failure to give 24 hour cancellation notice may incur a charge.
Our receptionists manage a large number of phone calls that come into the office each day. Early morning and early afternoon are particularly busy times of the day, especially on Mondays or after holidays. If your phone call is not of an urgent nature, please try to call mid-morning or late afternoon for best access. If you have an acute condition and need to be seen, please call the office and our receptionists will do their best to give you an appointment for that day.
In order to see your physician, you must make an appointment. It is necessary for our staff to ask questions regarding the nature of your visit in order to assess your needs and the urgency of your appointment. Our team members are trained to follow the policies provided by your physician.
There is always a “Doctor On-Call.” If you call your doctor’s office After Hours and you require medical attention please call 1-866-553-7205 to speak to a nurse. For continuity of care, we prefer that you do not use an After Hours Clinic or a Walk-In Clinic. After Hours Obstetrical Care – If you are an Obstetrical patient of Dr. Duncan please call (905) 338-4111.
Upon arrival for your appointment, please check in at the front desk with your health card. If your physician’s site has a self check-in kiosk you may use your health card to register your arrival electronically.
If you have any forms that you need the doctor to fill out, please make sure that all areas that pertain to you (e.g., name, health card number, address, signature, etc.) are filled out before handing them into the Business Office. Please note that there may be a charge for completion of the form. If you are unsure if there is a charge for the form, please refer to the price lists located in the display cases in the front and back lobbies and on the Business Office window. You may also call the Business Office at extension 116. Payment can be made at the Business Office when you pick up your form. We accept Cash, MasterCard, Visa, and Interact.
Can my doctor’s office email me?
There are some limits on what and when we can email you. The Caroline Medical Group can contact you by email to send you:
o Appointment bookings and reminders
o Referral bookings
o General information about our office and clinics
o Certain test results if your doctor has specifically discussed and agreed to this option
Please note: At this time, we do not receive emails from patients. We do not use email to exchange sensitive test results or share other clinical information.
Please call our office with a current email address and have a receptionist email you a Patient Consent and Release to Receive Emails form if you are interested.
