CLINICAL PHARMACIST

This position is to cover a Maternity Leave which will begin on April 1st, 2026.  This is a part-time position with 16 hours of work per week.  The location for the position is 3305 Harvester Road, Units 15-20, Burlington, Ontario L7N 3N2.

The Primary Care Team Pharmacist works within an interdisciplinary team to provide high-quality pharmaceutical care to optimize patient outcomes. The Clinical Pharmacist serves as a resource to the interdisciplinary team and is responsible for providing primary care to patients in accordance with the Standards of Practice of the Ontario College of Pharmacists as well as the policies and procedures of Caroline Family Health Team of Burlington.

The position requires a mature, cheerful person capable of professionally interacting with Patients, Healthcare Practitioners, Colleagues and Third-Party Individuals.

The role has a wide variety of duties and it requires effective organizational and communication skills.

Success in this position will be measured by accuracy and speed in managing the range of job activities, adaptability and ability to cope with changing priorities and stressful situations.

In this role, it is expected that the Clinical Pharmacist will behave in a confidential manner, consistent with our corporate directive.

Essential Duties and Responsibilities

Individual patient assessment to identify, prevent and resolve drug therapy problems. This includes conducting medication histories, medication reconciliation during transitions in care, deprescribing where appropriate, developing care plans and communicating the plan to the patient and interdisciplinary team.

Consultation based on referrals from interdisciplinary team members. Provide, on request, consultation to the physician and other team members to assist in the care of patients which may include functions listed above and/ or provision of drug information specific to the individual patient.

Provide drug information and academic detailing to physicians and interdisciplinary team members on pharmacotherapy and pharmacy related practice updates .

Enhanced provision of pharmacy services through the expanded scope for pharmacists including smoking cessation counseling and prescribing, adapting/renewing prescriptions, injection administration.

Recommend improvements to medication processes that will improve prescribing and medication safety and work with the team to implement them.

Contribution to ongoing clinical program development and evaluation.

Working Conditions

The ability to handle multiple tasks under pressure is important, as a CLINICAL pharmacist is the person ENSURING THAT REQUESTS FROM HEALTHCARE PROVIDERS, PATIENTS AND THIRD-PARTIES ARE HANDLED IN A TIMELY AND PROFICIENT MANNER. It is expected that the clinical pharmacist is able to work with all patients in a team environment. Within the team, errors are identified, acknowledged and corrected in a professional manner.

Requirements and Specifications

  • Registration in Part A of the Ontario College of Pharmacists is required;
  • Community or Hospital Pharmacy residency or equivalent clinical experience in either clinical setting is required;
  • Additional training or credentialing in pharmacotherapy (e.g., Certified Geriatric Pharmacist, Certified Diabetes Educator, Certified Asthma Educator, etc.) is considered an asset;
  • Demonstrated ability to work as a member of an interdisciplinary team;
  • Strong communication skills, both verbal and written;
  • Demonstrated commitment to continuing education.
