The Caroline Family Health Team is seeking a Psychiatric Nurse Practitioner to join our progressive team serving the residents of Halton on a full-time basis of 40 hours per week.
The Caroline Family Health Team provides a single point of access to health care services for all Halton residents. Our interdisciplinary care team offers comprehensive patient-centered, primary health care, including disease diagnosis, treatment and management, prevention and cure, rehabilitation, palliative care, and community health promotion.
As a multidisciplinary team member, the Psychiatric Nurse Practitioner is responsible for screening, diagnosing, and providing care for people facing mental health and substance use disorders, and working with primary care physicians and other health care professionals to coordinate care to patients in accordance with the Standards of Practice of the Nurse Practitioners’ Association of Ontario as well as the policies and procedures of Caroline Family Health Team of Burlington.
The PMH Nurse makes a diagnosis and collaborates with the patient and the health care team to develop a “plan of care” and evaluate it for effectiveness through follow-up. PMHNPs may also determine whether hospitalization or outpatient care is needed, or they may recommend individual or group psychotherapy. PMHNPs are also trained in crisis management, psychotherapy, and prescribing medications. Additionally, they are a resource in providing education to patients, families, caregivers, and communities.
PMHNPs treat a variety of patients, including teenagers and adults experiencing a mental health crisis or children who have been exposed to traumatic events. People confronting anxiety, depression, suicide, or those struggling with opioid, alcohol, or other substance abuse, can also be diagnosed and treated by a PMHNP.
Psychiatric Nurse Practitioners have a wide range of responsibilities, which can include:
- Communicating with patients, other physicians, and healthcare providers about their care plans
- Conducting evaluations to determine if a patient is ready to be discharged from the facility
- Evaluating patient’s mental and physical health needs, including interviewing them and reviewing their medical records
- Providing counseling services to patients who are coping with addiction, abuse, or other issues that may be contributing to their psychiatric problems
- Performing psychological evaluations to determine an individual’s ability to make decisions or participate in legal proceedings
- Providing health care services to patients with mental health disorders or substance abuse problems, including prescribing medications or providing therapy
- Working with the patient’s family to help them understand the illness and how they can support their loved one’s recovery
- Coordinating with other members of the health care team to ensure that patients receive appropriate care
- Performing administrative tasks such as maintaining patient records and managing budgets
Qualifications:
- Master’s Degree in Nursing required
- CNA certification in Psychiatric/Mental Health Nursing (CPMHN(c)), or commitment to complete within 5 years of hire as a condition of employment, is required
- Current BCLS/HCP certification required
- Current and valid registration and certificate in the extended class with the College of Nurses of Ontario
- Certification as a Primary health care NP in Ontario
- 1-3 years experience as an NP in a high-volume primary care setting
- Demonstrated ability to work independently within the RN (EC) scope of practice
- Experience with EMR and computer proficiency required
- Demonstrated commitment to continuing education
- Demonstrated skill at fostering cohesive interdisciplinary team function and effective communication skills
- Effective organizational skills, interpersonal skills, and communication skills (both oral and written)
- English proficiency is required
