The Caroline Family Health Team provides a single point of access to health care services for all Halton residents. Our interdisciplinary care team offers comprehensive patient-centered, primary health care including disease diagnosis, treatment, and management, prevention and cure, rehabilitation, palliative care, and community health promotion.

Social Worker is responsible for providing professional care to patients in accordance with the Ontario College of Social Workers and Social Service Workers as well as the policies and procedures of the Caroline Family Health Team of Burlington.

Essential Duties and Responsibilities:

  • Provide social-psychiatric case history information, psychosocial assessment, and social work treatment plans of assigned patients
  • Provide psychosocial support to patients and families affected by chronic, acute, or terminal illnesses
  • Advise family caregivers, counsel patients, and help plan for patients’ needs after discharge
  • Develop treatment plans with patients that engage clients to take an active part in their treatment
  • Consult with doctors, therapists, and medical professionals
  • Direct patients to other areas of assistance as required, and help patients and their families in finding services in the community
  • Instruct other healthcare professionals on the patient’s situation to ensure seamless care. Provide mental health counselling to clients and instruct patients’ families of the mental health conditions to strengthen their main support systems
  • Explain the social aspects of mental disturbances to families and community agencies involved
  • Maintain treatment records for each patient and prepare verbal and written reports and correspondence related to the work
  • Develop and implement programs as assigned
  • Work with other disciplines in diagnosing problems, formulating treatment plans, and evaluating progress
  • Provide social work treatment in accordance with medical treatment plans and modify plans to meet therapeutic goals
  • Attend scheduled department staff meetings
  • Attend required in-service programs

Requirements and Specifications:

  • Minimum of 3 years’ experience working in a primary care clinical team, registration with the respective College of Social Workers & Social Service Workers of Ontario in good standing & meeting legislative and College regulatory requirements to perform the controlled act of psychotherapy
  • Proof of professional liability insurance
  • Demonstrated ability to work independently as well as collaboratively within a multidisciplinary team
  • Excellent oral and written communication skills
  • Proficiency with electronic medical records and standard office software
  • Basic CPR certification and current Ontario Driver’s License

The Caroline Family Health Team is a Healthcare of Ontario Pension Plan (HOOPP) member. As a full-time employee, you will be registered in HOOPP during your first day of employment.
